All Digital Health Applications at a glance

A DiGA (Digital Health Application) is an app or web application that has been certified as a medical device and listed in the DiGA directory by the Federal Institute for Drugs and Medical Devices (BfArM). DiGAs help patients detect, monitor and treat diseases -- either independently or together with a doctor or therapist. They are also known as "apps on prescription" because, like medication, they are prescribed by a doctor and paid for by health insurance.

There are two ways: Either have a DiGA prescribed by a doctor or psychotherapist, or apply for one directly through your health insurance (provided you have a qualifying diagnosis). In both cases, you receive an activation code from your health insurance to unlock the app or web application.

For patients with a medical prescription, using a DiGA is completely free. All statutory health insurers in Germany cover the full costs. Many private health insurers also reimburse DiGAs. Some DiGAs additionally offer self-pay options -- you can find details on the respective DiGA detail page.

DiGAs can be prescribed by general practitioners, specialists and psychotherapists. No special prescription form is required -- a standard health insurance prescription with the DiGA name and PZN (pharmaceutical reference number) is sufficient. Alternatively, you can apply for a DiGA directly through your health insurance if you can provide proof of a qualifying diagnosis.

DiGAs are available for a wide range of conditions: Mental health conditions such as depression, anxiety disorders and sleep disorders make up the largest share. Other areas include musculoskeletal conditions (e.g. back pain, knee problems), metabolic diseases (diabetes, obesity), cancer, neurological conditions, cardiovascular diseases, tinnitus and more. Yes. All DiGAs must first be certified as medical devices (CE marking) and then go through the BfArM's fast-track process, which rigorously evaluates data protection, data security, interoperability and usability. The requirements are set out in the Digital Health Applications Regulation (DiGAV). Health data is processed exclusively in Germany or the EU.

Typically, all you need is a smartphone, tablet or computer with internet access. Depending on the DiGA, iOS (iPhone/iPad), Android or a web browser is supported -- the exact platforms are listed on each DiGA detail page.

The initial prescription usually covers 90 days. During this period, the DiGA should be used regularly so it can achieve its therapeutic effect. If needed, the doctor can issue a new prescription. The exact duration and recommended frequency of use varies by DiGA and is indicated within the app.

Some DiGAs offer self-pay options that allow use without a medical prescription. However, costs will not be covered by health insurance in that case. Information about self-pay options can be found on the respective DiGA detail page or on the manufacturer's website.

For provisional listing in the DiGA directory, the manufacturer must present a systematic data analysis that plausibly demonstrates an improvement in patient care, in addition to meeting the technical requirements. During the subsequent trial phase of up to 12 months (in exceptional cases 24 months), the manufacturer must then provide full evidence of a positive healthcare effect through a comparative study. Once this evidence is submitted, the DiGA receives permanent listing. For patients, there is no difference in use -- both types are covered by health insurance.
